Method of Payment:  A Demand Draft of Rs.225/- (As applicable) in favour of "Regional Director, ESI Corporation Indore drawn on any Nationalised Bank, Payable at Indore only.

How to apply: The application on A-4 size paper in the prescribed Performa duly filled in English or Hindi language, duly supported with clear and legible self attested copies of the relevant certificates and marks-statements (in English or Hindi) should be sent by Registered post so as to reach The Regional Director, Employees State Insurance Corporation, Panchdeep Bhawan, Nanda Nagar, Indore - 452 011 (M.P.) on or before closing date i.e. 31.07.2012, 5.00 PM. Candidates have to fill in the application form in their own handwriting with blue or black ball point pen. Original documents / certificates should not be sent with application. The candidate should submit one application only. The candidature of those candidates who submit more than one application is liable to be rejected. Envelope containing the application should be super scribed “Application for the post of Librarian Gr.II- 2012.” Applications not in the prescribed proforma as appended OR incomplete (without photograph) applications OR unsigned applications OR applications received after the last date of the receipt of applications OR without required enclosures are liable to be rejected and no reason of rejection will be communicated.
